A request does not need magic words
EEOC guidance explains that a PWFA request does not have to be in writing, use a particular form, or say “reasonable accommodation” or “PWFA.” That makes a dated personal chronology especially useful when the initial request happened in a conversation.
Track implementation, not just approval
An accommodation approved on paper can still be inconsistently implemented. The implementation log separates what was agreed from what happened on actual workdays and records whether the adjustment addressed the need.
