VA Symptom & Flare-Up Log
VA rates conditions on how often symptoms happen, how bad they get, how long they last, and what they stop you doing. A single appointment can't show that. A few lines a day can. Everything stays on this device: no account, no login, nothing sent anywhere.

What shows up most
No entries yet for this condition. Log the first one above — even a quiet day is useful, because "how often" needs the quiet days too.
This is a personal record-keeping tool, not legal advice, medical advice, or a claim filing service. GentleTools is not affiliated with the U.S. Department of Veterans Affairs or any government agency, and using it does not guarantee any claim outcome. VA decides what evidence it accepts and how it is weighed. For help with a specific claim, a VA-accredited representative, VSO, or attorney can advise you — many VSOs help at no charge.
Why the gap between appointments is where claims are won and lost
Rating schedules are built out of words like frequent, occasional, prostrating, near-continuous, and phrases about occupational and social impairment. Those are all measurements of a pattern over time. But the only thing that reliably lands in a medical record is the day you happened to have an appointment — and a lot of people, for entirely human reasons, downplay how bad things are while sitting in the exam room.
A contemporaneous log fixes that asymmetry. Written on the day it happened, it shows the flare you had on a Tuesday in February that never made it into any file, the three bad nights in a row, the week you used up your leave. It also gives you something concrete to bring to a C&P exam instead of trying to summarize two years from memory in twenty minutes.
Keep it honest in both directions. Log the good days too. A log that says "10 out of 10, every single day, forever" reads as less credible than one that shows a real pattern with real variation — and the real pattern is usually more than enough.
Also documenting the medical side? The Symptom & Function Diary covers daily functional limits, and the Doctor Visit Prep Sheet helps you get through an appointment without forgetting the thing you meant to say.
Frequently asked questions
Why does a symptom log matter for a VA disability claim?
VA rating criteria are written around how often symptoms occur, how severe they are, how long they last, and how much they interfere with work and daily life. A medical appointment captures one moment. A log kept over weeks or months shows the pattern between appointments, which is the part that is otherwise invisible in the record.
Can a veteran's own written account be used as evidence?
Statements from someone with firsthand knowledge are commonly referred to as lay evidence. A veteran does not need medical training to describe symptoms they personally experience and observe, such as pain, sleep disruption or difficulty concentrating. Whether and how any particular evidence is weighed is decided by VA, and an accredited representative or attorney can advise on a specific claim.
Is this log stored anywhere online?
No. Entries are saved in your own browser on this device using local storage. Nothing is uploaded, no account is created, and clearing your browser data removes the log. Export a backup file if you want a copy you keep yourself.
