Why keep the working record local?

Personal health records can be sensitive. MedNest runs as a browser-based file without a required GentleTools health account or automatic upload of the working record. That gives you more direct control over where the file and backups live.

The trade-off is equally important: GentleTools is not keeping a cloud copy for you. If the device fails, the browser data is cleared or you lose the recovery material for an encrypted file, there may be no vendor copy to restore. Keep independent backups in storage you control.

A quick daily function record

Record a severity score if useful and note which everyday activities were affected — work, driving, cooking, showering, sleep and other practical effects you choose to track.

A summary of what you entered

Review days recorded, user-entered severity, higher-severity days and how often selected activities were affected, with the dated entries behind the totals. It is a record summary, not an eligibility or claim decision.

Conditions and care-team references

Keep clinician-recorded condition names or codes you choose to copy, allergies, care-team contacts and equipment notes together. The app does not diagnose conditions or verify codes.

Medication history

Record current and past medications, doses, schedules, refill notes, side effects you observed and why you report stopping something. MedNest does not recommend, compare or evaluate medications.

Appointments, before and after

Write questions before a visit and record what you understood afterwards, including user-entered follow-up tasks. The app does not replace the clinician record or medical instructions.

Results you enter over time

Record selected values from source documents and chart them over time for your own reference. MedNest does not interpret whether a result is normal, abnormal or clinically significant.

Insurance and claim chronology

Keep plan references, billed amounts, user-entered balances, claim events, denial letters and contact notes together. The timeline organizes source information; it does not interpret coverage or decide an appeal.

Emergency reference page

Print a compact page from the allergies, conditions, current medications and contacts you entered. It is a reference sheet, not an emergency, triage or clinical service.

Optional local lock

If you enable the app’s local encryption feature, keep the passphrase and recovery material somewhere you control. There is no GentleTools password-reset service or cloud copy to recover the record for you.

Source information stays source information

If you copy a diagnosis, result, claim date, medication instruction or insurance figure into MedNest, keep the original clinician record, policy, notice or other source document too. The app can make those details easier to find and review, but it does not turn a user-entered note into an official medical or insurance record.
