IEP / 504 Accommodation Implementation Tracker

What the percentage means

The implementation rate counts only entries marked provided, partially provided or not provided. “Not needed,” “declined,” and “unknown” stay visible in the history but are excluded from the rate so they do not distort it.

Look for repeated contexts, not one bad day

A single missed accommodation can be noise. A recurring pattern around tests, one class period or one type of assignment is much more useful to bring to a team meeting.
