How to use the scores

The percentages are not grades. They are a way to compare domains and spot where the support plan is thin. A lower score in healthcare or self-advocacy may deserve more attention than polishing a domain that is already mostly independent.

Why student voice matters

Transition planning is about the student's adult life, not only the adults' checklist. Use this page as a conversation starter: compare the student's rating with the parent's or team's rating and discuss the gaps.
